The clinical activity focuses on hip diseases. Scientific and clinical lines: large diameter heads in ceramic in prostheses implants

The Orthopedics and Traumatology Unit of Policlinico San Donato is a highly specialized and innovative Unit thanks to the materials and the surgical techniques, especially in the treatment of traumatic and degenerative hip diseases and focuses on the orthopaedic and traumatology surgery for the hip.

 

  • Hip replacement surgery
  • Conservative prosthetic hip surgery (either arthroscopic or not) and hip traumatology
  • Traumatology surgery and chronic post-traumatic disease surgery
  • Arthroscopic operations on shoulder and knee and minor orthopaedic interventions.

Here the main interventions performed:

  • Hip prosthesis with sparing of tissues and using resistant materials (ceramic, titanium). Particular experience for complex cases (outcomes of: dysplasia and congenital dislocation and other growth diseases, traumas, previous interventions, inflammatory or neurological diseases)
  • Conservative hip surgery (not prosthetic) for young patients, which includes the treatment of:
    • Femur-acetabular impingement with mini-open and arthroscopic techniques
    • Congenital hip dysplasia, including congenital dislocation of children, adolescents and adults with femur and pelvis osteotomies;
    • Other growth diseases, such as Perthes disease, epiphysiolysis and their outcomes in adult age
    • Revision surgery: replacement of failed prosthesis because of consumed materials, mobilization, dislocation, ossification, infection or rupture with techniques including biological reconstruction of bone loss and restauration of stability, limb length and joint functionality.

  • Installation of non-cemented prostheses (different models depending on the hip morphology) and cemented prostheses in elderly patients or people affected by osteoporosis
  • Dual mobility prostheses for patients with increased risk of dislocation
  • Dedicated software for preoperative radiographic planning of the prosthetic implant
  • Preoperative CT scan for the study of the anatomy of the hip and the choice of the most suitable implant for each patient
  • Surgical instruments dedicated for the intra-operative control of limb length and offset
  • Application of modular implants with high capacity of integration with the bone (revision surgery)
  • Impaction grafting techinque in case of serious bone defects
  • Arthroscopic treatment and mini-open treatment in the femoral-acetabular impingement
  • Pelvis and femur osteotomy in young people with dysplasia or deformity

  • International multicenter study on the use of rivaroxaban for anti-thromboembolic prophylaxis in patients implanted with total hip prosthesis
  • Scientific production focuses on hip diseases and surgery.The hospital is a reference centre and produced consensus statements on the prevention of venous thromembolism in orthopedics and traumatology.

Various studies on femur-acetabular impingement, prevention of bleedings in prosthetic surgery, prosthetic materials longevity and prevention of venous thromboembolism have been published. Other fields are currently being investigated, especially new surgical approaches to the hip (anterior approach), minimally invasive methods (both prosthetic and hip arthroscopy) and on early diagnosis of infection in prosthetic surgery.
